THE WORK

Establish what happened

Bring together the accounts, records and relevant procedures. Build the chronology and identify gaps or contradictions without turning assumptions into findings.

Examine the contributing conditions

Look at decisions, interfaces, resources, procedures and controls. Consider how those conditions interacted and whether the same mechanism could cause another failure.

Define the response

Separate immediate actions from changes to the underlying system. Assign responsibilities and define how effectiveness will be checked.
